Please read the information below to make your appointments and follow-up care easier and more convenient:
- Save time! Download, complete and print your registration information to make your initial registration quick and easy.
- Insurances we accept. Note that we accept all insurances accepted by Porter Adventist Hospital, as they handle our billing.
- If you need referrals or refills, please notify your nurse at the beginning of your appointment so our staff can begin working on them while you are being seen.
- Co-pays are due at the time of the visit. Please bring your I.D. and insurance information.
- Patients that arrive more than 15 minutes late for physicals may be asked to reschedule, as they are more time consuming. Patients who are late for sick visits will be worked in between patients who have arrived on time.
- If a patient fails to keep an appointment without calling to cancel, they will be charged a $20.00 "no-show" fee, as our appointment spaces are limited. If a patient fails to show up for three appointments, they may be asked to seek a new provider.
